First Visit
We look forward to meeting you at your initial visit! This appointment will provide the opportunity for you to get acquainted with Dr. Aziz and several of our team members. We will take a series of photographs and perhaps a 3D image (CBCT) for immediate analysis. Dr. Aziz will review her diagnosis, applicable treatment option(s) and duration of treatment. Our Treatment Coordinator will then discuss professional fees, payment options and a treatment summary outlining the discussion will be forwarded to your Dentist and/or referring doctor.
Insurance and Payment Plans
Our Financial Coordinator is more than happy to assist you with all your insurance questions, complete applicable forms and arrange affordable payment options.
PIPA
Personal Information Protection Act
The purpose of the PIPA is to allow our office to share pertinent information with other healthcare professionals associated with the patient’s treatment.
The PIPA is a required document and must be signed prior to the initial appointment.
