First Visit
We look forward to meeting you at your initial visit! This appointment will give you the opportunity to get acquainted with Dr. Aziz and our team members. We will take a series of photographs and perhaps a 3D image (CBCT) for immediate analysis by the doctor. Dr. Aziz will review her diagnosis, different treatment options and duration of treatment, then our Treatment Coordinator will discuss professional fees and payment options with you. A treatment summary outlining our discussion will be forwarded to your Dentist and/or referring doctor.
Payment Plans and Insurance
Our Financial Coordinator would be happy to arrange interest free payment options and assist you with filing your insurance claims.
Personal Information Protection Act
The purpose of the PIPA is to allow our office to share pertinent information with other healthcare professionals associated with the patient’s treatment.
The PIPA is a required document and must be signed prior to the the initial appointment.
